FACT Act Consumer Disclosure Report

The Fair and Accurate Credit Transactions Act (FACT Act) was enacted in 2003 and amends the Fair Credit Reporting Act (FCRA), a federal law that regulates, in part, who is permitted to access your consumer report information and how it can be used.

The FACT Act entitles you to obtain one free copy of your applicable consumer report from certain consumer reporting agencies during a 12-month period.

1. Request Your FACT Act Disclosure Report Online

You can choose to request your FACT Act Disclosure Report online using our Online Request Form.

Once we have verified and processed your report request, you will receive a letter via U.S. Mail with details explaining how to access your report online.

2. Request Your FACT Act Disclosure Report by U.S. Mail

You can order your FACT Act Disclosure Report via U.S. Mail using the Printable Request Form.

Once you have downloaded a copy of the Printable Request Form, please complete all of the information and send the form back to us via U.S. Mail using the address below:
